Faculty Position Request Form
Division Heads (or their designee) must complete the Faculty Position Request Form to request a faculty position for inclusion in the SPH Annual Hiring Plan submitted to the Provost.
Submit one form per position request, unless part of an approved cluster hire. All submissions follow the SPH and University review and approval process.

Prep Document – Faculty Position Request Form FY27
Division Heads (or their designee) may use this prep document to draft responses for faculty position requests for the SPH FY27 Annual Hiring Plan. This document is for drafting purposes only. All official submissions must be completed in the online form.
Faculty Search Module Tool
The SPH utilizes Interfolio Faculty Search module/tool to manage all faculty recruitment and candidate evaluation activities. This system supports faculty search committees, administrative staff, leadership, and other stakeholders by facilitating a standardized, transparent, and efficient search process in accordance with University of Minnesota and SPH policies, processes, and procedures.
